Community Healthcare Services
Many older people prefer to live in a familiar environment with their family members and friends. However, in the event that the older person and/or the family cannot manage to care or support the needs of the older person, community healthcare services may be required to help the older person to remain at home.
These community healthcare services may be provided at home or in a centre.
Home based:
Healthcare services are provided within the homes of the older persons. The healthcare services would be medical, nursing or
palliative
care.
Centre based:
Healthcare services are provided within a centre. The older person will attend these centres during the day, usually on a regular basis, but will go back to their own homes for the night.
In order to qualify for government subsidies for these services, all patients will have to undergo
means testing
to determine the level of subsidies that the patient will get.
