Community Healthcare Services

Many older people prefer to live in a familiar environment with their family members and friends. However, in the event that the older person and/or the family cannot manage to care or support the needs of the older person, community healthcare services may be required to help the older person to remain at home.

These community healthcare services may be provided at home or in a centre.

Home based:Healthcare services are provided within the homes of the older persons. The healthcare services would be medical, nursing or palliative care.

Centre based:Healthcare services are provided within a centre. The older person will attend these centres during the day, usually on a regular basis, but will go back to their own homes for the night.

In order to qualify for government subsidies for these services, all patients will have to undergomeans testingto determine the level of subsidies that the patient will get.

Healthcare Services
Home Medical Service: provides medical care to the homebound older persons through home visits at their own homes.
Home Nursing Service: provides nursing care for housebound or bedridden older persons who require nursing procedures at their own homes.
Terminally Ill care and support for disabled sick (palliative care)
Home Hospice Medical & Hospice Nursing Service: provides medical and nursing care for housebound persons
Centre-basedHospice Day Care: provides social and rehabilitative programmes, during the day, for elderly suffering from advanced and progressive disease but yet staying with their family.
Out-Patient RehabilitationDay Rehabilitation Centres: provides centre-based rehabilitation services during the day, such as physiotherapy and occupational therapy to older persons who suffer from medical conditions that impair their functional status, so as to restore, improve and/or prevent any deterioration to their functional abilities.
Out-patient care for those with DementiaDementia Day Care Centre provides day care programme for older persons who suffer from dementia to teach and train them and their caregivers, so that the person with dementia can care for themselves as much as possible.
